Reconsideration Application Requirements

As required by Section 18(1) of the Social Assistance Act 13 of 2004, Sassa reconsideration forms must include the following information:

The applicant provides information about herself in Section A.

• A document proving the applicant’s identity. A person’s name, surname, age, and gender
